it's not really anonymous if you give your country/state/gender/age and you only get a small handful of people that you know to take it. I refuse take it because those are personal questions I don't wish to make public.

the last part should be radio buttons not check boxes. i checked them all. enjoy your useless data!

Do non-US responses actually help you at all?
And ditto on the radio buttons (and split sex/age into two questions) I recently did a hand survey which had multiple choices for a demographic question and no-one filled it out according to the instructions.
